A community health worker met a woman, who later became a client, at a Health Fair event. The client, who mainly spoke Spanish, explained that she had been diagnosed with diabetes five years prior, and that she no longer had a primary care physician. She said that even with medication her A1C was high, and she was looking for a way to improve her health.

The community health worker looked for a primary care physician for her in the safety net providers in Kansas City. After many phone calls, the client was got an appointment with a doctor at Samuel U. Rodgers Health Care Center. The client was able to create a health plan with her doctor, and also started seeing a nutritionist. With hard work, the client was able to lower her A1C!
