Discrimination Complaint Form

The Mid-America Regional Council assures that no person shall be excluded from participation in, be denied the benefits of, or be otherwise subjected to discrimination under any program or activity on the grounds of race, color, sex, national origin, age or handicap as provided by Title VI of the Civil Rights Act of 1964, Title IX of the Civil Rights Act of 1972, Rehabilitation Act of 1973 and Civil Rights Restoration Act of 1987. MARC further assures that every effort will be made to ensure nondiscrimination in all of its programs and activities, whether those programs and activities are federally funded or not.

The purpose of this form is to assist you in filing a discrimination complaint. You are not required to use this form. A letter containing the same information will be sufficient. It is important, however, to provide as much information as possible to assist in the resolution of the complaint.
