Living with a chronic condition such as diabetes, arthritis, heart disease, pain, stress or anxiety can be a daily challenge. But it doesn’t have to be! Find a course through the ma4 Network to help you manage chronic diseases, decrease stress and improve overall wellbeing. You can participate either in person or virtually.

Health Promotion Courses

Health promotion courses can help prevent disease or manage chronic diseases. These classes are evidence-based — they have been proven effective through research and evaluation. The classes provide tools to help you manage chronic conditions, develop healthy eating habits, create action plans for healthy living or focus on fitness and exercise. The courses also include self-maintenance plans to help you continue your new healthy habits after the class is over.

Social Programs

Social programs are fun, community-based cooking and nutrition classes aimed at improving your lifestyle and quality of life. You might even meet new friends! The MSN can connect you to a variety of activities and programs that can improve your life, including lifestyle and mindfulness programs, and exercise workshops.

*Medicaid Home and Community Based Program enrollees and caregivers can enroll in: Steps to Healthier Living (including Chronic Pain, Diabetes and Workplace editions), Tomando Control de su Salud, Walk with Ease, and Building Better Caregivers (Kansas City region only).

About the ma4 Network

The ma4 Network is an integrated network of Area Agencies on Aging (AAAs) throughout Missouri that aims to keep people healthy and independent in their communities for as long as possible through a variety of supportive, community-based services. The ma4 Network offers evidence-based workshops to equip Missouri residents with strategies to manage chronic diseases and improve their health and quality of life. 

The ma4 Network partners with a wide variety of agencies and community-based organizations to deliver workshops, with an Area Agency on Aging serving as the program lead in specific geographic regions in Missouri.

  • Aging Ahead (East-central Missouri) — four counties in Greater St. Louis
  • Aging Best (Central Missouri) — 19 counties in central Missouri and covering the northeast region for this project
  • Aging Matters (Southeast Missouri)
  • Area Agency on Aging — Region X (Joplin/Southwest Missouri) — four counties
  • Care Connection (Warrensburg area/West-central Missouri) — 13 west-central counties
  • Mid-America Regional Council (Greater Kansas City) — five counties in Greater Kansas City and covering the northwest region for this project
  • Missouri Association of Area Agencies on Aging (Statewide)
  • Senior Age (Springfield/Southwest Missouri) — 17 southwest counties
  • St. Louis Area Agency on Aging (City of St. Louis)
